General Practitioners & Consultants Registration with Hibernian Aviva Health

Hibernian Aviva Health require you to register with us for the following reasons:

To enable Hibernian Aviva Health to retain a list of Irish Medical Council Registered General Practitioners and Consultants
To enable Hibernian Aviva Health as required by taxation legislation to deduct Professional Services Withholding Tax

When submitting this to Hibernian Aviva Health, please also submit a copy (summarised) of your most recent curriculum vitae and your letter of appointment as a Consultant in Ireland

Please note that there a separate forms for a hospital/owns rooms/own surgery procedure and for an inpatient or home birth Maternity claim.
